This COAPT Risk Score Calculator is based on a publication authored by Neeraj Shah et al. in 2022 edition of JACC: CV Intv. This publication created a risk score using the COAPT Trial (ClinicalTrials.gov Identifier: NCT01626079) data to better stratify risk of 2-year death &/or heart failure hospitalization (HFH) for patients with severe functional mitral regurgitation (MR) treated with either transcatheter edge-to-edge repair with MitraClip or guideline directed medical therapy.

Calculators such as these are not intended to provide concrete treatment guidance for individual patients. The purpose of the calculator is to easily show which risk level an individual would fall into if they were one of the enrolled participants in the COAPT trial. Understanding that the calculator is based upon the COAPT trial patient population, it is expected that it will be most accurate if the exclusion and inclusion criteria of this trial are satisfied while using this calculator.

When weighing the risks and benefits of any treatment, the healthcare team must consider all available information on a patient’s medical history, preferences, and available devices. The significant factors identified through the COAPT sub-analysis that make up this calculator are not an exhaustive list of possible medical or comorbid conditions. It should be noted that patient population trends do not always apply in individual cases. Please use this calculator as an educational insight and an interactive means to understand the risk of death &/or HFH in patients with heart failure and functional MR.

  • New York Heart Association (NYHA) functional class?

  • Left ventricular ejection fraction (LVEF)?
  • Left ventricular end-systolic dimension [LVESD] (in cm)?
  • Estimated right ventricular systolic pressure [RVSP] (in mmHg)?
  • Tricuspid regurgitation (TR) grade?
